These are rarely required - Labels are typically formatted to fit a 2" x 4" label - Avery 8163 or similar.  

Labels Must include:

• Full Team Name - no abbreviations - Example, Hopkins PeeWee B1
• Player #
• Player First & Last Name (as it appears on official USA Hockey roster)
• Head Coach - First & Last Name, CEP # Level and Exp
• Assistant Coaches - First & Last Name, CEP # Level and Exp
