The Team Book ( Electronic File OR 3 ring binder) should be with the team manager (or someone filling in for you) at all D3 games, scrimmages, and tournament games.

Team Book should include:

• Official verified USA Hockey roster - will be emailed to you by HYHA Registrar
• Team roster with parent, names and contact info
• Any medical conditions that the coaches should be aware of (this should be kept confidential)
• Game Schedule
• Concussion Information (to give to parents in the event a player is suspected of having a concussion)
• Substitute Goalie request form
